Our office processes contact lens reorder forms Monday – Friday 8:30 am – 5:00 pm. Please fill out the form below and we will contact you for any follow up details we may need to fulfill your request. Payment for contact lenses held at our office is due upon pickup. If you would like your contacts mailed to you, a shipping fee will be added to all orders for less than a one year supply.
